What is Freed AI?
Freed AI is a HIPAA-compliant clinical documentation tool that listens to the audio of a clinical session and produces a written summary for your provider to review, edit, and finalize. It is used in this practice exclusively by Kristine Sinner for sessions conducted under the R.E.S.T.O.R.E. MNT program. It is not used by any other provider at Sinnergy Wellness Group.
Why your provider uses it
Manual documentation during a session divides a clinician's attention between you and the keyboard. An AI scribe allows Kristine to be fully present with you during your session and to complete high-quality clinical notes shortly afterward — while the details are still fresh. The result is more attention in the room, more accurate documentation after, and faster coordination letters to your treatment team.
How your privacy is protected
Freed AI operates under a signed HIPAA Business Associate Agreement (BAA) with this practice, meaning they are legally bound to protect your Protected Health Information (PHI) in accordance with 45 CFR Parts 160 and 164.
In addition to the baseline protections Freed provides, Kristine follows a proactive PHI-minimization protocol at the source:
- Your first and last name are not spoken aloud during the recorded portion of the session.
- Your full date of birth, address, and phone number are never entered into the audio stream.
- Names of family members and loved ones are not spoken. Family is referenced only by relationship — "your mother," "your brother," "your partner."
- If an identifying detail is accidentally spoken during session, it is redacted from the final note before it enters your chart.
- Only the reviewed and edited final note is placed in your Healthie chart. Draft transcripts are not retained beyond the clinical review window.
Where your information goes
The session audio is processed by Freed AI to produce a draft transcript and structured note. Kristine reviews, edits, and finalizes the note. The final note — not the raw transcript — is saved in your clinical chart in Healthie (also under a signed BAA). The transcript itself is handled according to Freed's data retention policy and is not shared beyond your provider.
Your rights
- Your consent is voluntary. You may decline use of the AI scribe at any time, and Kristine will document the session manually. Declining does not affect your care in any other way.
- You may withdraw consent at any future session — in writing or verbally at the start of a session — and your provider will stop using the scribe from that point forward.
- You may ask Kristine, at any time, to pause the scribe during a portion of the session you want to keep off the record.
- You may request a copy of your finalized clinical notes through the standard records request process described in the HIPAA Notice of Privacy Practices.
- You retain all other rights under HIPAA, including the right to file a complaint with the U.S. Department of Health & Human Services Office for Civil Rights at hhs.gov/ocr/privacy or 1-800-368-1019.
Limits of this consent
This consent is specific to Kristine's use of Freed AI for clinical documentation. It does not authorize recording of sessions for any other purpose, and it does not permit any other provider at Sinnergy Wellness Group or anywhere else to use AI scribing in connection with your care. It does not replace your Informed Consent for Treatment or the Notice of Privacy Practices you signed at intake.
